At Risk Registry

The Neenah Police Department At-Risk Registry provides first responders with information to assist in the care of those in a mental health crisis, those who cannot communicate effectively, or who cannot care for themselves. This information will provide first responders with the appropriate background information, communication methods and emergency contacts to better assist in the care of those in need.

If you have a family member who has dementia, autism, or other cognitive impairment, please complete the appointment form and submit it online. This form is also available at the Front Desk of the Neenah Police Department which is open daily from 7:00 a.m. to 11:00 p.m. Once submitted, our Behavioral Health Officer will contact you to make an appointment to obtain information needed for this registry.

Once added to our registry, annual follow up contacts will be made to update any information.
